Meet Our Case Manager
Peggy Shaughnessy
As a case manager I offer a combination of education and experience through years of working as a medical social worker and educator, working with individuals and families in their home, assisted living and hospital settings.

My role as case manager is to:

assess the social and emotional needs and concerns of clients and their family members

provide emotional support and guidance to individual and families
-
promote a client's continued well-being by educating clients, family members and care team as needed
-
advocate for both the client and their family to help navigate through a community system
-
share information regarding available community resources as needs arise
